I understand that every care has been taken to ensure that this procedure has been carried out in a hygenic manner and the aftercare of my tattoo is my sole responsibility

I understand that skin can have an allergic reaction to tattoo pigments and other liquids used during the procedure

I will follow the aftercare advice as explained to me by my tattoo artist

I have consented to the design supplied to me by my chosen artist prior to application

I understand that my artist is not responsible for the meaning of or spelling of any words or symbols of which i have chosen

I wish to preceed with the tattoo procedure and freely accept and expressly assume any and all risks that may arise from tattooing
